The Office of Safe Environment and Victim Assistance (OSEVA) has created two separate reporting forms for the mandated youth safe environment training. As a reminder, all training must be completed by the last Friday in December, and the reporting form must be turned in by the last Friday in January.


The two online forms managed by Cognito are NOT interchangeable:

  1. Parish Reporting Form
  2. School Reporting Form
