The OCYP Child Safe Environment Reporting Form is hosted by Cognito Forms. There are two forms that are now being used. The first is to be completed by the parish/school liaison which records the training that has been provided.
Training numbers for schools and parishes are to be submitted separately.
Parishes are to submit one form, which includes the combined numbers of Religious Education and Youth Ministry. The FY26 form may be found here:
Once the liaison has completed the form, in discussion with their Pastor or Principal, the form is submitted. A second form will be sent to the email provided in the "Email to route for approval" space on the form. The approval form is to be completed by the Pastor or Principal. OCYP will not accept hardcopies of the reports. To assist you, we have included .pdfs to provide a view before completing the forms.
